Te whiwhinga mahi | The Opportunity
We are seeking an experienced Health Navigator to join our Hospital WHIRI (Whānau Hauora integrated response initiative) rōpū, here at the Māori Health directorate Te Whatu Ora Waikato.

As a successful Whānau Hauora Assessment Navigator (Kaiwhiriwhiri) you will play a key role in supporting our whānau to planned care services clinic appointments, removing barriers to health care and closing the equity gap.

Day to day you will undertake activities as a Whānau Hauora Assessment Navigator under the Equity Hauora WHIRI projects. Engage with patients and their whānau, support patients to clinic appointments and conduct a Whanau Hauora Assessment. Follow up on identified areas of need as determined by the Whānau Hauora Assessment, ensure required referrals are made and appropriate education/resources are provided. Collect and enter data into screen tool. Escalate any concerns to Clinical Nurse Specialist.

This is a 1 FTE, permanent position where you will work Monday to Friday, with flexibility to work some days from home.

Key responsibilities of the role:
  • Support Whānau to clinic appointments
  • Conduct all Whānau Hauora Assessment activities in accordance with appropriate ethical requirements and high value care.
  • Complete all Whanau Hauora Assessments according to the protocol
  • Create and ensure referrals are sent to the appropriate organisations.
  • Ensure completed actions are documented. Prepare and ensure Te Whatu Ora, Waikato informed consent processes are followed.
  • Performing other duties considered to be within the scope of this role as agreed by you and your manager.

Mōu | About you
  • Previous experience as a health navigator
  • Work with whānau Māori and Pacifica
  • Great Communications skills and working within a team
  • Experience in the health sector
  • Māori development background
  • Proficiency in Te reo Māori me ōna tikanga
  • Drivers Licence
  • Mental Health Frist Aid Certificate
