Contract Length: 13 weeks-Accepting Local and Travel Candidates
Pay Rate: Range $1320 to $1400 Weekly Gross pay (Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Unit: Home Health
Shift: 8 Hours; Days, 8.00 am - 5.00 pm
Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri
Position Requirements:

• LPN experience: 2 years - Required
• Recent/Current Home Health Nursing experience: 1 year within the last three years - Required
• OASIS experience - Required
• Home Care Home Base (HCHB) charting experience - Required
• Must have valid Driver’s License and Personal Auto Insurance
• Must be Covid Vaccinated or provide medical or religious exemption

Education, License and Certification Requirements:

• ASN or an equivalent – Required
• BSN or an equivalent – Highly preferred
• LPN License for State of Florida or Compact - Required
• BLS Certification from AHA or American Red Cross - Required

Job Description:

• Provide and coordinate care under supervision for assigned patients in comfort of their homes
• Support patient recovery and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy
• Empower patients and families to reach healthcare goals by educating them on disease management
• Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
• Notify physicians of significant changes in status
• Document and complete charting for all patient visits within 24 hours.
• Territories to cover: Panama City and surrounding counties
