Summary:
A Targeted Case Manager specializing in foster care, adoption, and pediatric case management provides comprehensive support to children and families. This role involves coordinating services, advocating for the needs of children in foster care or adoption processes, and ensuring that healthcare, education, and social services are aligned to promote child well-being and stability.
Job Responsibilities:

• Assess the needs of children in foster care, adoption, or pediatric care, creating individualized care plans.

• Coordinate and monitor services such as healthcare, mental health, education, and social services.

• Advocate for children’s best interests in legal and social services settings.

• Collaborate with multidisciplinary teams including social workers, healthcare providers, and legal professionals.

• Facilitate family reunification, adoption processes, or placement in appropriate care settings.

• Conduct regular home visits, documenting progress and adjusting care plans as needed.

• Provide support and guidance to foster or adoptive families, ensuring they have access to necessary resources.

• Maintain accurate records in compliance with agency and regulatory standards.

Requirements:

• Bachelor’s degree in Social Work, Nursing, or related field.

• Prior experience in foster care, adoption, or pediatric case management.

• Strong knowledge of child welfare systems, community resources, and trauma-informed care.

• Excellent communication, organizational, and problem-solving skills.

• Ability to work independently and as part of a multidisciplinary team.

• State licensure or certification may be preferred.

Job Types: Full-time, Contract
Pay: $20.04 - $28.10 per hour
