Key Responsibilities :

Monitor and document patient health data, providing timely updates to healthcare teams.
Educate patients on health management, medication adherence, and wellness strategies.
Manage follow-up appointments and ensure continuity of care.
Inform patients of the result of validation of claims

Qualifications :

College Graduate
with Active PRC License
with at least 1 year Clinical/Bedside Experience
