Introduction: The Housing Navigator plays a crucial role in assessing the needs of individuals at our Mercy House Shelters, developing personalized housing plans, offering support and referrals, and advocating on behalf of residents. This role requires fostering strong, supportive relationships with residents to promote their independence. Keeping accurate and organized records is essential for this position.
Qualifications:
  • A Bachelor’s degree in Human Services or a related field is preferred. However, individuals with at least 2 years of direct experience working with long-term homeless, low-income, and diverse populations—especially those with knowledge of mental health and recovery—are encouraged to apply even without a degree.
  • At least one year of experience working with homeless individuals and mental health issues is preferred.
  • Must demonstrate high tolerance and empathy for clients with urgent, complex needs related to case management and health.
  • Strong verbal and written communication skills.
  • Ability to maintain a positive, professional, and safe environment while on duty.
  • Strong interpersonal skills for establishing and maintaining effective relationships.
  • Must hold a valid driver’s license, have reliable personal transportation, and provide proof of auto insurance.
  • Flexibility for evenings and weekends may be required based on shelter and navigation needs.
Duties & Responsibilities:
I. Service Delivery:
  • Manage a caseload of 22-30 clients, meeting with them as needed or per supervisor instructions.
  • Conduct initial navigation assessments within seven days of shelter entry.
  • Assess client eligibility and enter data into the Central Entry System (CES) for housing prioritization.
  • Assist clients in gathering required documentation for housing (e.g., ID, birth certificates, income verification, Social Security benefits).
  • Offer crisis intervention, referrals, and help with permanent housing placements.
  • Conduct comprehensive screenings to gather functional, financial, employment, housing, educational, and health information to create individualized service plans with both short- and long-term goals.
  • Actively participate in all phases of client service, including assessment, planning, engagement, and delivery.
  • Provide linkages to employment services, benefits, community providers for substance abuse, mental health care, and other needed services to help clients achieve housing stability.
  • Utilize harm reduction techniques to maintain housing stability for clients.
  • Offer crisis intervention services to support clients in becoming more independent and self-reliant.
  • Apply Trauma-Informed Care and Motivational Interviewing approaches when working with clients.
  • Ensure clients receive strengths-based case management and coordinated services to help them secure and maintain housing.
  • Collaborate with housing providers and community support systems, including outreach teams, housing authorities, landlords, social workers, and healthcare networks.
  • Assist with housing searches, application submissions, and reasonable accommodation requests, ensuring a safe living environment for move-in.
  • Enter accurate data into HMIS and maintain up-to-date client progress snapshots.
II. Team and Staff Relations:
  • Work collaboratively with the Mercy House team to implement the agency’s mission and core values.
  • Participate in regular internal and external agency meetings.
  • Contribute to the planning and development of resident programs.
  • Demonstrate flexibility and adaptability in a fast-paced, team-oriented environment.
  • Communicate effectively with diverse populations and maintain professional relationships.
III. Administrative Duties:
  • Maintain up-to-date, accurate documentation for all clients, including housing plans, case notes, and assessments.
  • Assist with data collection and ensure deadlines are met.
  • Be open to learning about Adult Rehabilitative Mental Health Services and Substance Abuse Treatment.
  • Utilize relevant software (e.g., Microsoft Office, Outlook, Excel, etc.) and systems necessary for the role.
  • Respond to calls and emails promptly.
  • Manage workload independently and prioritize tasks effectively.
  • Physical tasks may include lifting up to 25 lbs (e.g., accepting donations) and prolonged periods sitting at a desk working on a computer.
  • Perform other duties as assigned.
IV. Knowledge of:
  • Understanding of confidentiality, legal, and ethical issues related to case management.
  • Familiarity with documentation standards and guidelines.
