REPORTS TO: Health Services Outreach Manager
STATUS: Full-Time, Regular, Non-Exempt
COMPENSATION: $22-25 per hour

LOCATION: Sacramento, CA (this position is not eligible for remote work)

JOB SUMMARY: The Pr EP Navigator serves as a resource to support people seeking access to various health systems with a special emphasis on HIV prevention through Pre-exposure Prophylaxis (Pr EP) This position is an integral part of the sexual health team focusing on STI testing, harm reduction, prevention strategies, and community engagement. The Pr EP Navigator provides intensive care coordination and support services to HIV-negative and HIV-positive individuals who require assistance in accessing and remaining in Pr EP care. This individual will initiate access to Pr EP care support and continued access assistance to existing Pr EP patients.

ESSENTIAL FUNCTIONS:

Prevention
1. Nurture an atmosphere in which clients feel understood and seen by advocates who are aware of cultural competency concerns, health disparity issues, and access to care deficits.
2. Provide clients with education regarding Pr EP/PEP, screen for readiness, and assess other barriers to care, working with the client to identify position solutions and support their needs.
3. Conduct healthcare provider education regarding Pr EP and other biomedical intervention strategies to prevent the transmission of HIV.
4. Provide HIV/STI pre- and post-test client-centered counseling, HIV/STI testing, risk assessments, and risk reduction and prevention coaching/counseling.

Case Management
1. Make referrals to external service providers and assist clients with scheduling visits to further discuss Pr EP and other health care concerns with a medical provider.
2. Conduct follow-up to offer adherence counseling and assistance in scheduling appointments for patients receiving Pr EP; In some cases, in consultation with the Pr EP coordinator, provide transportation to and from Pr EP appointments
3. Assess benefits and insurance, and if needed, aid access payor programs including acquiring and maintaining required documentation for patient assistance programs (PAP), Pr EP Assistant Programs (Pr EP-AP)
4. Effectively collaborate with all individuals involved in Pr EP care and program delivery to support high-quality Pr EP programming.
5. Enter data promptly, per agency guidelines, and ensure confidentiality of client information.

Community Outreach and Engagement
1. In collaboration with Development team, create innovative outreach and marketing strategies and targeted advertising campaigns to reach those at risk and may otherwise avoid more traditional care settings, linking them with primary care physicians and increasing internal program participation.
2. Participation in outreach activities including Pride, health fairs, and other local activities.
3. Provide sexual health education, resources, materials, and referrals via peer group session facilitation to inform LGBT+ individuals about safer sex practices and high-risk behaviors.
4. Recruit high-risk patients and clients into Pr EP education.
5. Provide outreach to relevant communities with a focus on men who have sex with men, transgender women, and BIPOC communities.
6. Establish working relationships with appropriate community resources in the area, including those providing mental health and social services to the target population.

General Duties:
1. Serve as a collaborative member of the Center’s Health Programs Department and be able to participate in daily activities and functions to maintain the overall Program’s operations.
2. Facilitate or participate in staff meetings, team meetings, and other meetings as required.
3. Collaborate with sexual and gender health stakeholders from the county and state.
1. Maintain appropriate patient files and timely documentation of services utilizing electronic prevention or medical software.
4. Handle a variety of special projects and other related duties, as assigned.

QUALIFICATIONS AND EXPERIENCE:
1. Experience working with the LGBTQ+ community and familiarity with issues of relevance to LGBTQ+ people.
2. Excellent interpersonal, verbal, and written communication skills.
3. Demonstrated ability to work effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, socio-economic backgrounds, religions, ages, English-speaking abilities, immigration status, and physical abilities in an intersectional environment.
4. Availability: Monday-Friday 9:30 a.m.-6:00 p.m; occasional evenings and weekend hours as needed, or flexible schedule to include regular evening and weekend hours.6:00 p.m.; occasional evenings and weekend hours as needed, or flexible schedule to include regular evening and weekend hours.

REQUIREMENTS:
1. Passion for the Center’s mission and work to create a region where LGBTQ people thrive and a commitment to follow all policies and procedures of the organization.
2. Education: Bachelor's degree in public health, social service, or related field and 1-2 years of experience in case management; or at least 3 years of relevant direct experience may be substituted for formal education.
3. Experience: Experience (paid or volunteer) in HIV/STI testing and counseling, prevention services, or supporting people living with HIV/AIDS is preferred.
4. Certifications in CPR, phlebotomy, and HIV Basic I Counselor are preferred.
5. Ability to speak/read/write/translate English. English is required; bilingual Spanish is preferred.
6. Computer/Office Equipment Skills: Excellent proficiency in working with Microsoft Office, particularly Outlook, Word, Excel, Power Point, One Drive, and Teams. database systems, and social media tools.
7. Access to reliable automobile transportation with a valid driver’s license and proof of insurance is required. This position may on occasion be required to drive rented vehicles and/or our Mobile Outreach RV.
8. Physical Demands: The person in this position may be required to sit or stand for extended periods, move objects up to 50lbs., ascend/descend stairs, operate office equipment, open and close filing cabinets, and boxes, observe visitors, and communicate messages by telephone.
9. Offer of employment may be contingent on satisfactory results of a criminal history background check.
10. Employees must be legally permitted to work in the United States.
11. The Sacramento LGBT Community Center is a drug-free workplace, employees may be subject to a drug test.
12. Vaccination Requirement: All employees are required to be vaccinated for COVID-19 and are encouraged to stay up to date annually or obtain an approved medical or religious exemption. Documentation will be required before your first day of work.
