Supported Housing Specialist

Your role in action:
- Assess initial consumer needs, based on individual strengths and available resources.
- Maintain and monitor an assigned caseload of consumers, through casework/resource management and referral, and skill teaching/refinement to help them obtain and maintain housing.
- Participate in developing goals with the consumer, for inclusion in the individualized service plan.
- Ensure provision of preventative healthcare and necessary follow-up services.
- Provide case management and support to consumers residing in Supported Living sites and establish or reinforce links to the local community on behalf of the consumers.
- Conduct home visits of assigned caseload. Assess and identify gaps in service delivery and recommend alternative action plans.
- Maintain and submit required program reports and documentation, including progress notes, service plans, and incident reports.
- Serve as an advocate on behalf of the consumers and encourage them to assume a proactive role in their rehabilitation process.
- Utilize local resource networks for referral to enhance program continuity
Education :
- Bachelor's degree from an accredited institution in social sciences field or related field (psychology, sociology, etc.)
- Prefer minimum of 1 year of experience working in the field of mental health.
- Basic understanding of mental illness, common signs and symptoms and able to identify difficulties with coping in community settings.
- Strong communication skills, listening and interpersonal skills above average, written documentation is clear, concise, well- constructed and grammatically correct. Able to work collaboratively with others in a teamwork environment.
- Effective time management and organizational skills. Able to execute standard word processing software on the computer.
