About the Role
The purpose of this position is to support Aboriginal and Torres Strait Islander people through the implementation of a range of chronic disease management activities through the application of the principles of best practice, education, and leadership to ensure safe practice and high-quality client outcomes.

• Engage with the local Aboriginal and Torres Strait Islander community and relevant health and social services to identify priority actions to improve the health outcomes of Aboriginal and Torres Strait Islander people and their families.
• Develop, as required, communication and engagement strategies with key stakeholders and annual action plans which identify strategies to address priority issues for the target population.
• Deliver evidence based and culturally appropriate individual clinical services and group health promotion, education and Chronic Disease management activities to Aboriginal and Torres Strait Islander people and families.
• Promote the importance of physical activity, rest/sleep, being mentally healthy, good nutrition and reduction of preventable Chronic Disease risk factors (smoking, alcohol, obesity).

This is an identified vacancy with a genuine occupational requirement for the position to be filled by a candidate indigenous to the Aboriginal or Torres Strait Islander Community. Candidates not meeting this criteria will not be considered.
