About the role:
Join a dynamic role where each day brings new challenges and opportunities! In this position, you’ll engage in case management, coordinate patient care, and travel across the district to visit patients in hospitals and at home. You’ll also have the chance to provide valuable education to facilities, services, and individuals. We offer a structured induction programme and monthly clinical supervision to support your development, along with opportunities to enhance your knowledge through a variety of face-to-face and online programmes. Collaborate with a diverse range of teams and services within HNELHD, as well as private hospitals and aged care facilities, making a real impact across the state. About you:
  • You are an exceptional communicator, particularly in discussions about death, dying, and conflict resolution.

  • You are resilient and demonstrate integrity while working autonomously and as part of a multidisciplinary team, with a recent background in acute medical or palliative care.
