The Geriatric Memory Support Unit provides care for patients who are medically stable and are waiting transition to a community Residential Aged Care Facility. This unit is located at the Repat Health Precinct and has a strong emphasis on a partnership between the patient, family and community. The multidisciplinary clinical care team develops and enacts support plans to provide tailored patient activities to meet individual needs and effective discharge planning.
The Registered Nurse/ Midwife reports to the Nursing Director of Rehabilitation, Aged and Palliative Care and supports the Geriatric Memory Support Unit with person-centred care consistent with regulatory and statutory requirements.
About the Role:

Assess individual patient/client needs, plan, implement and/or coordinate appropriate service delivery from a range of accepted options including other disciplines or agencies
Provide ward/team leader/coordination as required on a shift by shift basis
Participate in quality improvement activities that contribute to patient/client safety, risk minimisation and safe work activities within the practice setting
Use foundation theoretical knowledge and evidenced based guidelines to achieve positive patient/client care outcomes
Plan and coordinate services with other disciplines or agencies in providing individual health care needs

To be successful for this role:

Effective communication skills including, problem solving, conflict resolution and negotiation skills
Ability to work effectively within a multidisciplinary team
Registered or eligible for registration as a Nurse and Midwife by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate
Demonstrated competence in rehabilitative and geriatric nursing and or midwifery practice in accordance with the relevant standards
