Senior Occupational Therapist (Level 3) – Community Transition Aged Care Program (TACP)

About the Role
The position is a community role, covering the Sydney Local Health District geographical area. TACP has its office base at Concord Hospital, but you may be asked to commence or finish work from other SLHD locations which include RPA, Canterbury and Balmain Hospitals. There may be opportunities for mobile work arrangements and community based work-related activities will require use of your own car.
The SLHD Community Transition Aged Care Program (TACP) serves to transition aged care clients safely from hospital back into the community. The TACP Senior Occupational Therapist, in conjunction with the multidisciplinary TACP team, will work with clients to achieve their goals and ensure safe discharge from the program. The Senior Occupational Therapist also supports and mentors other Occupational Therapist staff in the team.

Ideal Candidate

Bachelor or post graduate degree or equivalent in Occupational Therapy or equivalent, with experience at level 2 or higher. Current registration to practice with AHPRA and current NSW Drivers Licence (P1 and P2 acceptable).
Demonstrated advanced clinical knowledge, reasoning, skill and significant community clinical multi-disciplinary experience in the provision of Occupational Therapy in the assessment and treatment of older people in the community.
Demonstrated high level organisational, negotiation and time management skills in an environment of competing clinical/administrative demands.
