The Licensed Practical Nurse (LPN) handles the delivery of patient care through nursing processes. You will provide in-home direct patient care according to the provider's prescribed plan of care (and nursing scope of practice) while closely monitoring the progress/recovery of the client. The Licensed Practical Nurse (LPN) works in accordance with agency philosophy, standards of professional nursing practices, and all federal and state laws and regulations, including HIPAA as applicable.
Job Expectations:

• In home, skilled nursing visits could consist of (head-to-toe assessment, vitals, medication management, wound care, disease process education)
• Implements plan of care initiated by the Registered Nurse.
• Provides accurately and timely documentation consistent with the plan of care.
• Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.
• Demonstrate knowledge in medical care; such as diagnostic procedures, medication, symptoms, and other treatment-related therapies.
• EMR documentation (AXXESS) & (Axiscare)

Qualifications:

• Graduate of an accredited practical nurse or vocational nursing program.
• Active nursing license in the state of Indiana.
• Complies with accepted professional standards and practices.
• Strong written and verbal communication skills.
• Tech Savvy
• General knowledge of Home Health practices, 2 years preferred.

Expected hours: 9 – 18 per week
