Schedule: 40 hours min. per week - IN PERSON, Staten Island, NY

Reports To: Director of Prevention Family Services and Supervisor of SBES
Pay: $58,500 annually maximum (*based on city, state and/ or federal contractual obligations and budgetary allowances):

The Division of Prevention Services partnered with UAU and Staten Island schools to offer families flexible service options and support. This is part of the broader interagency efforts between ACS and the DOE to prevent families at risk of receiving ACS interventions at home.
POSITION SUMMARY: STATEN ISLAND, NY - The Case Planner is responsible to manage an average caseload of about 12 to 15 families at any given time. The Case Planner will conduct home visits and meet with families to assess the families' strengths, the safety, risk and well-being needs of children and the assistance that families may require. The Case Planner reports directly to the Supervisor of SBES and Program Manager. The Case Planner is expected to effectively communicate, provide appropriate documentation as outlined below and adhere to deadlines as set forth by the Director of Prevention Family Services. The Case Planner is responsible for providing and coordinating excellent quality care and healing to UAU’s participants and ensures that every one of the children on the caseload is living with a nurturing and safe, environment. The worksites for the Case Planner will include but are not limited to; UAU’s Admin Office, NYC Public School(s), participant households or other locations deemed appropriate according to UAU Administration while maintaining professionalism in all areas, including confidentiality. Bi-lingual REQUIRED - Spanish OR Mandarin

PRIMARY JOB RESPONSIBILITIES (include but are not limited to):
- Conducting safety and risk assessments for families at risk of losing their children to foster care.
- Maintain a caseload of 12 families.
- Create individualized service plans for each family focused on reducing assessed risk.
- Facilitate Collaborative Service Case Planning conferences every 6 months per family.
- Support families with connection to community resources, financial entitlements, mental/physical health services, educational supports, etc.
- Provide casework counseling in supporting families toward reaching their service goals.
- Maintain contact with families including conducting monthly in-home visits.
- Timely completion of documentation including assessments, case contact notes, etc.
- Stay current on prevention best practices and attend 42 hours per year of mandated training to continuously improve program effectiveness.
- Work alongside families to understand their goals, values, beliefs, and cultural background.
- Develop and deliver support plans that empower families to keep their children safe and healthy in a way that aligns with their unique needs.
- Facilitate access to resources and services within the community to strengthen families and promote well-being.
- Assist with the development of collaborative partnerships with the school staff, administration, and stakeholders.
- Maintain and manage daily and monthly proper and effective communications with any applicable stakeholders, school staff, participants and families pertaining to any type of event, activity or any other related SBES programming and initiatives.
- Responsible to ensure that all documentation related to any and all SBES participants and families are kept secure (under lock) and confidential.
- Other duties as assigned.
- Bachelor’s Degree in Social Work or related field required with a minimum of 6 credits in Education, Psychology or a related field.
- Minimum of 1-year case management, assessment, counseling or related work experience required.
- Bi-lingual REQUIRED - Spanish OR Mandarin
- Excellent communication skills, both verbal and written.
- Strong computer/technology skill set, including proficiency with Microsoft Office Suite (Word & Excel), CONNECTIONS and PROMIS ACS.
- Ability to work in a collaborative environment and accept constructive criticism.
- Obtain and maintain all security clearance requirements throughout the duration of employment.

- Must be culturally competent - the ability to understand, be sensitive to the needs of, communicate with, and effectively interact with people across all cultures.
- Familiar with the regulations and requirements for ACS and NY State Departments of Health and aware of the compliance requirements as enacted by Federal, State and local jurisdictions as applicable based on the department’s contractual obligations.
- Required; Valid drivers license and reliable, accessible mode of transportation
- Willing and able to speak, read and translate Spanish, preferred
- Willing to take initiative and work a flexible schedule when needed
- Excellent organizational and time management skills

- Incumbent will be scheduled based on operational need.
- Standard office equipment including computers, fax machines, copiers, printers, telephones, etc.

- Travel is required to a variety of locations, based on programming needs. Which is reimbursed in accordance with UAU’s fiscal policies and procedures. Reliable mode of transportation required.
- May include working prolonged periods of time sitting at a desk and working on a computer.
- The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.
- The ability to observe details at close range (within a few feet or more of the observer).
