Reports To: Director of Prevention Family Services and Supervisor of SBES

The Division of Prevention Services partnered with UAU and Staten Island schools to offer families flexible service options and support. This is part of the broader interagency efforts between ACS and the DOE to prevent families at risk of receiving ACS interventions at home.
POSITION SUMMARY: STATEN ISLAND, NY - The Case Aide is responsible for the providing assistance, coordination and support to the team of Case Planners, at the direction and supervison of the Supervisor within the School Based Early Support (SBES) department. The Case Aide is will be responsible for providing quality support and assistance for the families we serve while maintaining a consistent and positive rapport between both staff and families. Bilingual REQUIRED - Spanish OR Mandarin
PRIMARY JOB RESPONSIBILITIES (include but are not limited to):
· Support Case Planners in service goal planning and outcomes.
· Support families with connecting to community resources, financial entitlements, mental/physical health services, educational supports, etc.
· Provide support to families toward reaching their service goals.
· Maintain contact with families including conducting monthly in-home/virtual visits.
· Timely completion of documentation including assessments, case contact notes, etc. Including maintaining an accurate family appointment/field visit calendar.
· Participate in planning of community events and activities.
· Liaison for community agencies and school resources
· Responsible to ensure that all documentation related to all SBES participants and families are kept secure (under lock) and confidential.
· Assist with the development of collaborative partnerships with the school staff, administration, and stakeholders.
· Maintain and manage daily and monthly proper and effective communications with any applicable stakeholders, school staff, participants and families pertaining to any type of event, activity or any other related SBES programming and initiatives.
· Performs other related duties as necessary or assigned
JOB QUALIFICATIONS:
· Bachelor’s Degree in Social Work or related field required with a minimum of 6 credits in Education, Psychology or a related field, preferred.
· Bilingual REQUIRED - Spanish OR Mandarin
· Excellent communication skills, both verbal and written.
· Strong computer/technology skill set, including proficiency with Microsoft Office Suite (Word & Excel), CONNECTIONS and PROMIS ACS.
· Ability to work in a collaborative environment and accept constructive criticism.
· Obtain and maintain all security clearance requirements throughout duration of employment.
· Demonstrated ability to conduct and complete work independently
· Willing to take initiative and be flexible when needed
· Excels working in a collaborative environment to achieve target objects and outcomes
PHYSICAL REQUIREMENT:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. · Incumbent will be scheduled based on operational need.
· Standard office equipment including computers, fax machines, copiers, printers, telephones, etc.
· Position is in a well-lighted office environment.
· Involves sitting approximately 50% of the day, walking or standing the remainder.
· Must be able to remain in a stationary position for at least 50% of the time.
· May include working prolonged periods of time sitting at a desk and working on a computer.
· The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.
· Ability to lift 25lbs, as requested and needed.
· The ability to observe details at close range (within a few feet or more of the observer).
