This role involves caseload management and co-ordination, clinical decision making and accountability for highly complex patients/clients. You will work closely with all members of the Home Team, including Social Work, Public Health and Allied Health Professionals.
Applicants will ideally have the qualifications to fulfil the requirements of this role, namely V300 independent and supplementary prescribing and advanced clinical assessment. However, this may be a development opportunity for a registered Nurse to undertake this role and be supported to complete the 2 aforementioned modules.
The Community Nursing Service operates an open access policy available 365 days a year. The service does not operate on a waiting list; care needs are prioritised and the team respond to planned and unplanned care.

NHS Scotland is reducing their full-time working week from 37.5 to 37 hours per week for NHS Agenda for Change workers in Scotland from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period, for which you would be paid, until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
