Care Coordinator, Springwood Community Mental Health Team

Nepean Blue Mountain Local Health District provide a range of specialised community mental health services that support people to live well in their communities. We work within a clinical framework that promotes recovery-orientated practice and supported decision-making.

The Springwood Community Mental Health Team is a small team of dedicated Mental Health Clinicians providing mental health assessment, interventions and care coordination for adults experiencing mental illness and disorder. The service operates between 8.30am and 5.00pm Monday to Friday and flexible work practices can be negotiated.

As a Care Coordinator, you will work collaboratively with our multidisciplinary team and stakeholders to provide the highest level of evidence based clinical care which will be collaborative, client centred, recovery orientated, trauma informed. Your role will ensure the inclusion of nominated carers, you will encourage self-advocacy and facilitate empowerment and capacity for optimal independent function.

What you will bring to the role
  • Tertiary qualifications in Nursing and including eligibility for membership of relevant professional body & full registration with AHPRA (including completion of a new graduate RN program or equivalent experience working as a Registered Nurse)
  • Demonstrated skills in mental health and discipline specific assessments and interventions and experience working with people with mental illness
  • Demonstrated ability to collaborate meaningfully with consumers and carers with a commitment and understanding of recovery principles in mental health service provision
  • Demonstrated working knowledge of NSW Mental Health Act, 2007
  • Demonstrated excellent communication skills including verbal, written, and good interpersonal skills
  • Computer competence and literacy
  • Current NSW drivers licence and willingness to use it for work purposes
