An exciting position has arisen for a Nurse Practitioner or Transitioning Nurse Practitioner within our dynamic Central Coast Hospital at Home (CCH@H) Team. This is an amazing opportunity to show case your skills and make a real difference in the provision of services to the older population across the Central Coast.

About You
  • You have Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Agency (AHPRA) with endorsement from the Nurses and Midwifery Board Australia to work at an advanced level as a Nurse Practitioner in Aged Care Nursing or working towards as per the NMBA Guidelines on endorsement as a Nurse Practitioner.
  • You have the ability to provide advanced nursing care in Aged Care, independently conducting assessments, investigations, and implementing appropriate treatments or referrals in line with advanced nursing practice guidelines.
  • You possess strong communication and negotiation skills, along with proven experience in using computerised health information systems.
Where You’ll Be Working

The Nurse Practitioner/Transitional Nurse Practitioner (NP/TNP) in Aged Care plays a pivotal role in delivering advanced and extended clinical care. Working collaboratively, the NP/TNP ensures safe, high-quality nursing care to older adults through comprehensive assessment, management, and treatment. Their focus is on providing clinical support to older individuals, contributing to the enhancement of healthcare systems within the Local Health District and Residential Aged Care Facilities (RACFs), ultimately aiming to reduce unnecessary hospital admissions and support early discharges.

Key responsibilities include the delivery of safe, specialised care plans tailored to the needs of older adults, developed in collaboration with medical teams and General Practitioners. The NP/TNP will also maintain close coordination with the Hospital in the Home services on the Central Coast.

In this role, the NP/TNP leverages their expertise to assess and manage the health needs of older adults, which may involve initiating diagnostic investigations, prescribing medications, and referring patients directly to other healthcare professionals. All practices are conducted within the NP/TNP's approved scope, ensuring alignment with legislative, policy frameworks, and the strategic goals of the Central Coast Local Health District (CCLHD).
