- Southern Adelaide Local Health Network - Mount Gambier and Districts Community Health Centre - DASSA Outpatient Services
- Salary $121,537.00 - $126,970.00 RN/M3 (Pro Rata) | Part time 16 hours P/W - Short Term contract - Up to 12 months
Duties for the role will include:
- Undertaking assessment of hospital patients in response to requests by admitting teams.
- Communicating treatment management plans to all relevant members of the health care team including health workers in the community.
- Collaborating with the health care team, facilitating the implementation of the treatment management plans.
- Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
- Demonstrated ability to work independently and self-direct to prioritise workload, set timelines, manage workload demands, maintain accurate records, assess and analyse problems and formulate solutions to achieve objectives.
- Effective written and verbal communication skills including problem solving, conflict resolution, negotiation and working effectively within a multidisciplinary team
