The Office of the Chief Clinical Officer (CCO) was established as part of an overall investment by the HSE to strengthen governance and accountability for the planning and delivery of high-quality services with the aim of driving transformational change across our healthcare system through clinical leadership, design of new models of care, promotion of a culture of safety and quality improvements; through patient and service user involvement.
The Clinical Design and Innovation (CDI) function, reporting to the National Clinical Director of Integrated Care, within of the Office of the CCO provides Clinical Innovation, Design Leadership and Direction to strategic decisions across the wider organisation.

The role will be based and managed through the RCSI National Clinical Programme for Surgery and its governance structures. The RCSI National Clinical Programmes are a strategic initiative between the HSE and the various postgraduate training bodies.
The programmes aim to design and implement change initiatives to improve and standardise the quality of care and access for all patients in a cost-effective manner.
The Programmes are structured with broad cross-functional input. Clinical leadership is provided by the training bodies to ensure that the patient remains at the centre of any change recommendation.
Scope and Purpose of Post
Neurosurgery is a major surgical specialty that currently does not have a workstream within the National Clinical Programme Structure. The benefits of having a National Clinical Lead for Neurosurgery and adopting a programmatic approach to support clinical pathway planning, service development, governance and strategy development will be important for the development of the specialty.
The appointment of a National Clinical Advisor for Neurosurgery aligns with the current structure within NCPS and will work with established governance both through the RCSI committee for surgical affairs and through the National Clinical Advisor and Group Lead for HSE Acute Operations.
Background
2235 operative surgical discharges are recorded for Beaumont, CUH and CHI Temple Street Hospitals for 2023 with an additional large number of non-operative neurosurgical discharges from these and other general hospitals nationally. Beaumont is by far the largest neurosurgical unit and provides many of the national neurosurgical subspecialty services including epilepsy surgery, skull base, vascular, spine and trauma and has the largest major neuro intensive care unit caring for critically ill patients.
General Duties of the Post holder
- Define the current state and develop future service needs which include resource requirements based on national gap analysis
- Develop standardised Care Pathways which are quality-assured to support Primary Care, the Community and other Acute Hospitals which are patient-centric for the patient journey in line with Sláintecare
- Identify key risk and mitigation plans to stabilise the service and support training and education needs for succession planning in supporting and collaboration with the RCSI Training Programme Director
Specific Expectations of the post holder
- Develop Neurosurgical pathways, protocols and guidelines for the shared services with the MMUH, Major Trauma Centre
- Develop a Model of Care for Neurosurgery in Ireland which incorporates the requirements for the Trauma Report for Ireland
- Develop a workforce plan to support the implementation of a MOC
- Develop a resource plan and associated business cases to support the implementation of MOC
- Develop relevant guidelines and protocols to support the implementation of a MOC
- Develop recommendations on the governance of Neurosurgery services and their integration with other relevant specialties
- Develop recommended KPIs for a Neurosurgery service.
Waiting Lists
- As of 1/2/24, there are over 10,663 patients on the OPD waiting list which is a 21.*% increase on 2023. Neurosurgery is the sixth-highest surgical specialty for inpatient waiting list numbers. Part of this role is to work to find solutions and coordinate the effort to better manage the waiting lists and provision of services.
Integration of Neurosurgical Services with the Trauma Network
- It is expected that the post holder will lead on the engagement to map new patient pathways, protocols and guidelines necessary for the integration of Neurosurgical services with the MTC in MMUH
Parliamentary Questions
- Respond to parliamentary questions relating to Neurosurgery that are forwarded to NCPS within the agreed HSE timeframes.
CAAC Approvals
- Provide an opinion on any new or replacement consultant posts for Neurosurgery for the CAAC approvals process.
Working Group participation
- Participate in working groups or provide opinions as the need arises for NCPS

- The National Children’s Hospital development
The role will involve active membership of the NCPS Steering Group and other groups and committees which support the work of the Programme including but not limited to leading in specific initiatives, whether at the HSE or DOH level, such as waiting lists, unscheduled care, hospital configuration, surgical manpower and surgical workforce planning.
Working closely with the other Clinical Advisors, the Programme Leads, Programme Managers, Programme Steering and Working Groups, the Managing Director of Surgical Affairs and the oversight committee for NCPS which is RCSI CSA, and with the support of the will contribute to prioritising, initiation and leadership of work-streams and projects relevant to the NCPS.
Reporting Arrangements
Reporting to:
- RCSI Committee for Surgical Affairs (the RCSI Clinical Programmes oversight committee) or its designate (Managing Director of Surgical Affairs) and through the Clinical Leads of the National Clinical Programme in Surgery (NCPS) and the specific Clinical Advisory Group (CAG) that will be formed to guide this role,
Working closely with:
- National Clinical Advisor Group Lead (NCAGL), Acutes
- Clinical Design & Innovation (CDI), Office of the Chief Clinical Officer (CCO).
The Clinical Advisor shall ensure that the CDI is provided with all necessary information to exercise governance and policy responsibilities. This reporting relationship will be facilitated by regular meetings between the Programme Manager, the Co-Leads, the HSE NCAGL for Acuts and RCSI. In line with Clinical Design & Innovation progression and/or restructuring, the reporting relationship may change.

Leadership
- To act as the lead clinical advisor for Neurosurgery services to develop and implement a framework for Neurosurgery services on a national level.
- To provide expert clinical advice, leadership and practical support towards a framework for national Neurosurgery services to ensure consistency, reliability and validity of care.
- Provide support and direction for RCSI in the development of sustainable trainee programmes.
Other
- Make information available in respect to PQs or Reps for Government as required.
- Adhere to the HSE Communications protocol for any interaction with the press or media and in relation to any public relations events or queries.
- Adhere to the HSE National Financial Regulations for any expenditure or costs associated with the programme of work.

Eligibility Criteria
All staff of the HSE, other statutory health agencies, the Department of Health and bodies which provide services on behalf of the HSE under Section 38 of the Health Act 2004 are employed as registered Medical practitioners currently employed at Consultant or Clinical Director level with a minimum of 5 years experience, as a Consultant in an Acute setting who are working within the specialty of Neurosurgery are eligible to apply.
Applicants must be registered with the Medical Council of Ireland on the specialist division of the register.
