The successful candidate will be a Support Worker within the Integrated Outreach Team .

The service aims to: Improve and maintain our service user’s engagement with services.

Our people will have a primary problem of psychosis and many will experience problematic co cooccurring substance use.

We aim to provide trauma informed care and in so doing, promote and support the recovery needs of our service users.

The post holder is expected to work as a member of the Integrated Outreach Team under the direct or indirect supervision of a qualified nurse whilst fostering the teams and organisational philosophy and approach to care delivery. The post holder has responsibility to feedback the outcomes of care delivered daily to the team.
