Administration Officer - Transition Home

About the role
In late 2023, Activ opened a new transition service in Bunbury that focuses on providing support for those living with disability and requiring mental health support, as they make the transition from long term stay in hospital to home.
As the Administration Officer, in this position you will provide in-home support to the Transition Home Manager and our dedicated team of support workers, with regular interactions with our six customers, their families, and other local providers.
The typical duties in the role may look like:
- Assisting with the capture and completion of support plan information and documentation
- Compiling data for mandatory reporting, and ensuring audit and compliance requirements are met
- Building agendas and providing accurate and prompt minute taking that can be shared with stakeholders
- Document management including uploading, filing and archiving
- Effective and timely communication across multiple platforms, including; email, phone, calendar, inbox and webex
This is a part time position working 19 hours a week (.5 FTE), with a minimum of 4 days, and will pay $33.61 per hour (equiv. $66,400) plus salary packaging options. Some flexibility may be needed around start and finish times due to service requirements.
The ideal candidate will have:
- Previous administration experience in a similar industry or setting
- Strong computer skills, accustomed to working across multiple platforms with a focus on accurate completion of documentation
- Able to take a proactive approach to administration duties, and quickly change tasks when needed
- Ability to show discretion and maintain a high level of confidentiality
- Empathy and resilience to work in the support environment, with the willingness to engage with our customers and staff and live the Activ values.
