The Transition Care Program (TCP) provides short-term care that seeks to optimise the functioning and independence of older people after a hospital stay. Transition Care is goal-oriented, time-limited and therapy-focused. It provides older people with a package of services that includes low intensity therapy such as physiotherapy and occupational therapy, as well as social work, nursing support or personal care to maintain and improve physical and/or cognitive functioning.
Transition care at CHRH is delivered on behalf of Grampians Health and the Victorian State Government Department of Health. Bed based transition care is provided at Creswick Hospital, Daylesford Hospital and Trentham Hostel and community-based TCP is provided in the clients home within the Hepburn Shire.
We are seeking an experienced Registered Nurse or Allied Health Clinician with excellent assessment, organisational and critical thinking skills to join our Central Highlands Rural Health at home team as a Transition Care program - Case Manager. The successful candidate would ideally have previous experience in Transition Care Programs, Complex and Chronic disease management programs, Hospital in the home or related specialist program streams. They would also demonstrate a strong commitment to highest quality patient care and delivery of best patient outcomes.
Details of Appointment
Fixed term contract working 32 hours per fortnight (6 months from date of commencement).
Salary:

Registered Nurse: YY4 Clinical Nurse Consultant @ $54.64 per hour as per EBA + 11.5 % superannuation + access to salary packaging
Allied Health Clinician - Dependent on experience / as per award + 11.5% Superannuation + access to salaray packaging

Central Highlands Rural Health (CHRH) is a rural health organisation that operates five campuses across Hepburn Shire and the Macedon Ranges in the Central Highlands of Victoria in Australia, north and north-west of Melbourne. It was formed through the voluntary amalgamation of Hepburn Health Service and Kyneton District Health on the 30th November 2019. Each of our campuses retains the name of the township in which they are situated - Clunes Health, Creswick Health, Daylesford Health, Kyneton Health, and Trentham Health.
