Are you a dedicated and compassionate nurse looking to make a real difference in regional South Australia? Country Health Connect is seeking a skilled Community Health Nurse to join our vibrant team in Mount Gambier.
Joining the Country Health Connect team, you will provide high quality primary health care to clients living in the Mount Gambier community. Utilising your interpersonal skills and ability to build strong relationships, you will assist with the provision of professional nursing care aimed at improving patient health and clinical outcomes. With the opportunity to use a variety of skills, your duties will involve assessing patients’ needs, planning, implementing and coordinating appropriate service delivery options and communicating changes in condition and care. You will be working in partnership with consumers, communities and other organisations in the Limestone Coast Region, as part of a multidisciplinary model of care.

You must be registered or eligible for registration as a Nurse or Nurse/Midwife with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate
